Payment PolicyWe require payment at time of service. For your convenience, we have several different billing plans. Insurance Coverage:We are happy to bill all insurance plans, provided we have current insurance cards at the time of service. Because your policy is a contract between you and your insurance company, it is your responsibility to know and understand your plan’s requirements and policies regarding co-payments, co-insurance, or deductible amounts. Should your insurance carrier deny a clam, we will make a reasonable attempt to help you resolve the disputed issues. If, however, we have provided your carrier with all the required information to process your claim and they persist in refusing to reimburse for that claim, the responsibility for resolving the dispute will be yours. Should you make an overpayment, your account will be credited with the overage and a refund will be provided as long as there is no outstanding balance. Please allow 10-14 business days for refunds to be processed and mailed to you. Secondary Insurance Coverage:Our office will bill secondary insurance carriers as a complimentary service for our patients. We strongly encourage all our Medicare patients with secondary insurance coverage to arrange automatic claims forwarding by Medicare to your secondary carrier if such arrangements are possible. This allows us to keep your health care costs under control by reducing unnecessary paperwork and employee time to bill secondary carriers manually. No Insurance Coverage:Patients with no insurance are self-pay patients. We encourage all self-pay patients to make arrangements in advance to pay for the care you will receive. For all office visits, we will expect payment at time of service. For all non-emergent or elective surgical procedures, we require 30% of our charges to be paid in advance. We will work out a payment plan for the remainder of the balance. Any surgical procedure that is paid in full within 90 days of service will be eligible for a 20% discount. We encourage all self-pay patients to see if they qualify for a government funded health care program prior to seeing us. Government Funded Coverage:Otherwise known as Valley Wide CICP. We will offer a 50% discount of surgical services if patients provide us in advance of their appointment all appropriate paperwork. We also require 50% of charges for non-emergent or elective surgery to be paid in advance of the surgical procedure. We also require office visits to be paid at the time of service. For those patients that are receiving financial benefits from government agencies, we are required by law to collect appropriate co-payments specific to your agreement and the agency involved. Debt Management Coverage:We will not sign an agreement with any debt management agency. We accept payment by cash, check, VISA, and MasterCard. Failure to Pay:Your account is considered delinquent when there is an outstanding balance and no payment activity or payment arrangements 90 days after receipt of insurance payment for any service. Self-pay patient accounts will be considered delinquent if no payment activity or payment arrangements are made within 90 days from date of service. All delinquent accounts over 90 days with no payment or arrangements made will be subject to collection proceedings. This includes the account being turned over to an outside collection agency. ***We are happy to provide a copy of this policy upon your request |
